Grave Purchase Fee = $700.00/Per Lot

Perpetual Care Fee = $500.00/Per Burial or Cremation Burial with a check sent to Jim Potter 10656 S. Howell Ave. Oak Creek, WI 53154.

• Winter flowers to be removed from graves by April 1 of each year

• Summer flowers to be removed by October 1 of each year.

• Dog burial/or dog cremation is not allowed.

• Cremation burial shall be staked out at the same charge as a regular burial fee.

• Two (2) cremations are allowed to be buried on one (1) gravesite and stone to be placed and marked accordingly.

• Any adjustment of gravestone is the responsibility of the grave lot owner.

• After 3 to 5 months of a burial the excavation area shall be leveled and sodded or seeded with charge being part of the $500 Perpetual Care Fee.

• It will be the lot owners responsibility to hire a grave digger with help from funeral director for any regular burial or cremation burial.

• The concrete base of any monuments placed shall be not less than 5”around perimeter of monument.
